Question about the two penalties rule

Friend of mine told me a story at the weekend of the worst shot he'd ever seen. Chap stands on the first tee with his driver. Takes a swipe at the ball and hits it right off the inside of the heel. The ball goes between his legs, hitting them in the process and continues on until it finishes out of bounds. Question is, when he retees his ball, is he playing his fifth shot or his third. I presume it's his fifth, but I'm not sure if that would count as being penalised twice for one infraction.

He's penalized 2 shots for the golf ball hitting his equipment or himself. He's also penalized for the ball going OB. He'd tee up another ball and be hitting his 5th shot.
