It's entirely possible, of course, but I don't think that's necessarily it. I can tell when I'm setting up too close. It is the obvious answer, but I was just wondering what the other possibilities are.

A few possible causes that come to mind, leaning back at impact, lifting up at impact, not hitting down on the ball, having the club face open too much, outside swing path, or ball position.

I know when I'm doing this, I'm coming more from the outside. I try to tell myself to keep my arms close to my body and let the club return to the ball from the inside. I usually make much crisper contact in the center of the club face.

Try pushing the club back with your left hand (if you are right handed) and then pull it back through with with the same hand. While doing this, make sure to keep your body extremely still and watch the ball very closely. Most people I see pull the club back with their dominant hand and then push it back through, which results in an outside to in swingpath. Hope this helps.

You are too close to the ball at impact so you are either too close to the ball at set up or you are moving toward the ball during the swing. Most people crowd the ball on chips and pitches, so that would be my bet. Your set up with a pitch should be the same as with a full swing with theat club. If yours is different then you have been setting up too close to the ball.
